Chiropractic Assessments

Before an Initial Consultation, a new client assessment form will need to be filled out. This gives your chiropractor vital information about your medical history, lifestyle and any issues you may have which could impact your current state of health. The form is completed online (no printing necessary) and takes 10-15 minutes to fill out.

Different forms are available via the same link for infants, toddlers, children and teenagers, pregnancy, and clients transferring from other clinics. Please choose the appropriate form for you.

You also may be asked to fill out a progress survey by your chiropractor. This allows your chiropractor and, more importantly, you to evaluate the progress you are making under your care with us.
